Dental Membership Plans
Helping patients enjoy optimal oral and dental health is our paramount priority at Ridgeline Family Dentistry. That’s why we’re pleased to offer patients three dental membership options to make our care even more accessible. These are Standard Care, Gingivitis Care, and Perio Maintenance Care.
All three are in-office discount dental plans that allow individuals without insurance to receive dental care at our practice for a discounted price. Each of the three plans is an annual plan for individuals and/or families.
- Eligibility: Anyone without insurance is eligible!
- Enrollment: Enrollment can take place at any time. This is an annual plan that is paid in full at time of enrollment. No refunds are available for this plan.
- Payments: All co-payments and procedure fees will be due at the time of service.
Each year your membership is active you are entitled to the following:
- Two professional dental cleanings per year*
- Two doctor exams per year
- Routine X-rays
- Fluoride treatment (two per year)
- Oral cancer screening
- All other dental procedures at a 10% discount off our standard fee. (Some exclusions may apply.)
Annual Fee
Cost: $468 per person per year/$408 per child (under 12 years old) per year
Your year begins when you sign up. Please note that these fees are non-refundable.
*defined as routine cleaning only, not advanced hygiene services such as scaling and root planing, full mouth debridement.
Each year your membership is active you are entitled to the following:
- Three gingivitis therapy cleanings per year
- Two doctor exams per year
- Routine X-rays
- Fluoride treatment (two per year)
- Oral cancer screening
- All other dental procedures at a 10% discount off our standard fee. (Some exclusions may apply.)
Annual Fee
Cost: $642 per person per year
Your year begins when you sign up. Please note that this fee is non-refundable.
Each year your membership is active you are entitled to the following:
- Four perio maintenance therapy cleanings per year
- Two doctor exams per year
- Routine X-rays
- Fluoride treatment (two per year)
- Oral cancer screening
- All other dental procedures at a 10% discount off our standard fee. (Some exclusions may apply.)
Annual Fee
Cost: $756 per person per year
Your year begins when you sign up. Please note that this fee is non-refundable.
Terms and Plan Limitations (applies to all three membership plans)
- Effective date is the date you sign up; renewal is every 12 months.
- This is a discount plan, not dental insurance. It cannot be combined with dental insurance or other offers.
- This plan is only good at Ridgeline Family Dentistry.
- The plan is non-transferable. Family members cannot be substituted in for another family member.
- This is a non-refundable plan. No refunds are given if the patient does not use the entire plan value.
- Payment for treatment is due at the time of service. If care credit is used, the discount is reduced by 5% due to merchant fees.
- A fee will be charged for each broken appointment without two (2) business days advance notice.
- The discount does not apply to products, Invisalign, Zoom Whitening, laser treatment, or specialist’s care.
